OCD and Anxiety Center of Cleveland Informed Consent for Text (SMS) Messaging Form
As part of our services, and with your prior express written consent, we would like to send text (SMS) messages to the mobile number you list below in the consent section of this form.
Text (SMS) Messaging Purpose and DescriptionTerms and Conditions

By opting in to receive SMS messages from OCD and Anxiety Center of Cleveland (“we,” “us,” “our”), you agree to these Terms and Conditions (Terms).
SMS Messaging Service
By providing my phone number, I consent to receive SMS text messages from OCD and Anxiety Center of Cleveland for appointment reminders, and other logicalal reasons with The OCD and Anxiety Center of Cleveland. Message and Data Rates may apply.
Message Frequency
You will receive more than one message from us unless you opt out. While messaging frequency varies, you will likely receive messages pertaining to your sessions or scheduling around 4 times per month. We reserve the right to alter the frequency of messages at any time to increase or decrease the total number of messages. The company and carriers are not liable for delays or undelivered messages.

Cancellation/Opt-Out Instructions
You can opt out of receiving SMS messages at any time by replying STOP to any message we send you. After you opt out of text messaging, you will receive one additional message confirming your request has been processed.

Liability
We are not responsible for any charges, errors, or delays in SMS delivery or undelivered messages caused by your carrier or third-party service providers.
Privacy and the Health Insurance Portability and Accountability Act HIPAA
We utilize a phone and text service that is compliant with HIPAA to increase privacy when communicating via phone or text. However, no system of communication can be considered 100% secure and there is always a risk of unauthorized access if the encryption system is breached. Additionally, while we take significant steps to protect your information (HIPAA compliant services, password protected apps and password protected phones), we cannot control unauthorized access to your phone. Should you lose your phone or it be accessed by an unauthorized user, they would be able to read any texts that you have sent us.
Given these risks, we utilize text messaging primarily for scheduling and other logistical issues. We do not consider it a clinical tool and you are advised not to send clinical information or questions via text message. Direct messaging (regarding clinical concerns) with Dr. Lakin is available to individuals who become clients of the OCD and Anxiety Center of Cleveland via our secure client portal. The purpose and limitations of this direct messaging system will be discussed with you at the time of your first session.
In case of Emergency:
Our text and phone messages are never to be used for emergency purposes. If you are having a psychiatric emergency , you may get immediate assistance by calling the 24-hour Suicide Crisis Hotlines at(800) 273-8255 or by texting “START” TO 741741 or you may contact 911, your physician, or psychiatrist.
